I need to see your full name and birthday. You may cover any other details you're not comfortable sharing.

Employment Verification
Your name, phone number, email, and a selfie of your face while touching your nose — plus one or more of: LinkedIn profile (photo, job listed, 80+ connections), email from your work address (can be blank), photo of your work ID badge, or social media with your real name.

References / Okays
TOB, P411, or other references. Your name, phone number, email, and a selfie touching your nose. Via P411: send an appointment request so I can see your recent Okays. Via TOB: share your references.
Screening is required for all new clients — this is non-negotiable. I assure you that discretion and privacy are of the utmost importance to me. Screening can be done several ways; please select whichever method suits you and include that information in your initial message. Including everything in your first message greatly expedites the process.
